i acknowlede that participation in PB Circuit Training classes & personal training sessions involves risk of injury or even death. I voluntarily assumes all such risks.

I hereby release, waive, and discharge Patrick Bexfield & PB Circuit Training from any and all claims, liabilities, damages arising from my participation. This includes any claims for personal injury, property damage or death, expect where caused by gross negligence.

I certify that i am physically able to participate in this activity and have no medical conditions that would prevent safe participation.
